All our staff undergo security screening as part of an Enhanced DBS (Disclosure and Barring Service) check and are carefully vetted for identity and right-to-work.

How does care in the home work?

Home care not only allows patients to be in their own home and receive needed health care services, but it can also provide support to family and carers. We can also provide doctors with information on the patient’s condition in between doctor’s appointments for a full and proactive home care service.

Home care may seem a daunting prospect for both the loved one at home and their relatives. However, it’s quite simple really once you know some facts. You can then make informed decisions and decide the best approach for you.

The first step is to call us. We can chat through your specific circumstances and we can then explain our services and how we might be able to help. There is never any hard sell with SD Care. The essence, core and success of our business rests with the wellbeing of you or your loved one. It’s our Franciscan tradition.

If you think we can help, then we can book a no obligation home care assessment. This merely focuses on a package of care that is right for you so you can make informed decisions about the future.

A Care Plan

As we said, no two clients are the same. We will certainly suggest many things, but fundamentally this is ‘your’ care plan. We will listen to your needs and wants and build something to meet your wishes. If there is anything that needs explaining, then we shall ensure it is fully understood. There may be some really useful items that will lift your quality of life and make your home care plan something very positive.

We can discuss and arrange dates, times and responsibilities, but if this becomes too much for one visit, then we are happy to let you mull over the key information before progressing. Essentially, we set out who will be coming and when, and what you can expect at every stage. A weekly rota is agreed. As life isn’t always straightforward, we build lots of flexibility into your care plan, and even if you decide that it’s not the best solution for you, then you can stop with only a week’s notice within the first month.

We adjust to suit you

Once you are up and running, we are always on hand to adjust your care plan to suit. We have a scheduled review every 3 months where you can change things.
